Who We Are
The CDS Wolf Foundation, named in honor of CDS Monarch founders, Lew & Phyllis Wolf – seeks donations to help support the growth and expansion of CDS Life Transitions’ community services and unfunded areas of need. Established in 2007, The CDS Wolf Foundation aims to help make a lasting impact on the lives of individuals with intellectual and developmental disabilities, seniors, and veterans.
Residential and community living services, temporary housing for veterans, affordable housing, clinical services, case management, special olympics, recreation activities and emergency assistance are some of the unfunded areas that the CDS Wolf Foundation supports.
Our Commitment

Gifts to the Foundation go directly to providing critically needed opportunities that provide support to people assisted by CDS Life Transitions.

He is the Executive Director of CDS Monarch, with responsibilities for all day-to-day operations as well as administrative functions. As Chief Executive Officer for CDS Life Transitions, he supports all affiliates through strategic leadership.
